Both Isildur's and Shagrat's text would activate before Shagrat is placed in the discard pile.
BTW
With All Possible Speed is as optional as Shagrat's text. As are all response actions.
The mandatory part is taking a wound, just like with Shagrat the madatory part is loosing a skirmish.
EDIT: But I see you have edited your post so this doesn't make a whole lot of sense now.
